FORMAL COMPLAINT

Dear Ms Davis,

I wish to make a Formal Complaint about Danebridge Medical Practice's plan to close Sandiway Surgery.

I am a patient who has used the surgery regularly for (xx) years.

I believe that this growing village needs a place where a patient can see a GP or health professional when they need to.

Travelling into Northwich to see a GP is not an option for a number of patients in this rural area and the loss of a surgery here can only be detrimental to your patients.

I object strongly to the plan to close Sandiway Surgery.
